In addition, vaccination data can be used by schools and health departments to help during a response to an epidemic by identifying those students most at risk of disease, and allowing them to be best protected from exposure. **

*What if circumstances do not allow my student to be vaccinated? *
Vaccination Exemption forms are an option and can be completed during a visit with your student's healthcare provider. If you are choosing to exempt your student from the required immunization schedule, please request your health care provider sign, then fax this form to (833) 965-0909. Like vaccination records, a current exemption form must be on file with the school to better help during a response to an epidemic by identifying those students most at risk of disease, and allowing them to be best protected from exposure.
